General Instructions

If you received a notification from Acadia Health d/b/a Just Kids Dental (“JKD”) that your personal information was or may have been compromised in the "Data Incident” discovered in or around August 2023, then you may be a Settlement Class Member. Included in the Settlement Class are three subclasses: the Minor Subclass; the Adult Subclass; and the SSN Subclass.

To know what subclass you are apart of, please review the settlement Notice sent by the Settlement Administrator. If you are unable to locate your notice or did not receive notice and believe you are part of the Settlement Class, please contact the Settlement Administrator.


Available Benefits

If you are a member of the Minor Subclass: Members will automatically be provided up to 10 years of Cy Ex’s Minor Defense Pro product (or something substantially similar). No other relief will be available for Minor Subclass members and no claim needs to be filed to receive this benefit.

If you are a member of the Adult Subclass: Members can make a claim for Documented Ordinary Losses up to $2,500, Alternatively Adult Subclass Members can claim a Pro Rata Cash Payment in lieu of claiming for documented losses. Members can only get a payment if they fill out a Claim Form, if the settlement is approved, and if are found to be eligible for a payment.

If you are a member of the SSN Subclass: Members can make a claim for credit monitoring and identity restoration services, as well as claiming for ordinary and/or extraordinary documented losses up to $10,000 collectively. Alternatively, SSN Subclass Members can claim a Pro Rata Cash Payment in lieu of claiming for documented losses. Members can only get a payment if they fill out a Claim Form, if the settlement is approved, and if are found to be eligible for a payment.

To start your online claim form, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on your email or postcard notice. If you are unable to locate your ID & PIN, please contact us by emailing info@JKDDataSettlement.com. Please request your Unique ID & PIN by providing the Class Member’s full name and mailing address.

Claims forms must be submitted online or postmarked by August 6, 2025.
